If a medical condition means you do not meet the DVLA requirements at the time of your examination, our doctor will explain the findings, advise you on the next steps and, where appropriate, recommend review by your GP or specialist before a reassessment. Patient safety and DVLA guidance remain our priority.
You can request your medical records by:
- Using the NHS App
- Emailing/handing our GP Full Medical Summary Access Request Letter (Download here and fill in with your details)
- Photo ID (Driving Licence or Passport)
- Proof of address (e.g. council tax bill etc)
- DVLA D4 form
- Glasses or contact lenses
- Medication list
For Taxi/Private Hire Vehical Drivers
- Medical Decleration form
- Full GP Medical Summary
Yes. We offer same week appointments, including evenings and weekends, to fit around your work schedule.
We explain why they have failed. We provide appropriate information on how to successfully complete medicals and next steps. Where appropriate, recommending further assessment or treatment through the patient's GP or specialist. We also offer a discounted repeat medical once the issues are resolved.
